A Supports Coordinator works closely with the individual in need of support and assistance to help them receive the best possible care and resources. They work as case managers linking the individual they serve to the appropriate services and supports. Their primary role includes locating, coordinating, and monitoring services accordingly for the intellectually disabled individual.
The Role of the Supports Coordinator
- Ensure that the Behavior Support Plan and ISP are consistent with Initial and Quarterly intake of individuals seeking and receiving services in the home and or community.
- Ensure services are based on desired goals and needs.
- Has monthly contact and quarterly contact monitoring visits in settings where individuals are receiving services and communicates when there is a crisis or other related services, issues, and/or concerns.
